Important guidelines you need to know. Please Read!

We will call you to confirm that your Online Check-In information has been received, then call you again when we are ready for you to come in to be treated. If you have not heard from our office within 30 minutes of submitting your form online, please contact our office at 733-9400.

Online Check-In is Urgent and Family Care’s innovative process that virtually eliminates waiting in the clinic lobby.

With Online Check-In you can register at home, at the office, or anywhere you have online access.

Since there are no appointments made at Urgent and Family Care, the Online Check-In form is not an actual appointment.  It’s a convenient way to check in and be called as soon as your room is ready.

Our office is only able to process Online Check-In forms up to 60 minutes prior to closing. If you submit this form during a time which we are closed, the form will not get processed until the next business day.

Warning:  Please dial 911 if you have an emergency such as chest pains, uncontrolled bleeding, abdominal pain, fracture with bone showing, severe burns, poisoning, constant vomiting, serious breathing problems, or anything else you deem as life threatening. Online Check-In is not for emergency treatment.

Some patient emergencies may alter the order in which Online Check-In patients are seen.
